The RCVS has brought out a new publication to help members of the veterinary and veterinary nursing professions understand their legal and professional obligations regarding controlled drugs.
Controlled Drugs Guidance is a consolidation of existing advice and guidance on controlled drugs from a number of organisations including the RCVS itself, the Veterinary Medicines Directorate (VMD), the British Small Animal Veterinary Association (BSAVA) and the Home Office. The guide also gives details of the existing legislation concerning controlled drugs, namely the Misuse of Drugs Act and associated regulations.
The guidance provides advice on a variety of topics to reflect the common queries the RCVS Advice Team receives, such as storage, destruction and disposal; keys and keyholders for controlled drugs cabinets; controlled drugs in vehicles; and veterinary nurses administering controlled drugs.
The publication also deals with specific controlled drugs such as ketamine which, on 30 November 2015, was rescheduled to a Schedule 2 controlled drug, meaning that it is now subject to the same strict storage, prescription, dispending, destruction and record-keeping requirements as other medicines in this Schedule.

Guillaume Fournié and Dr Javier Guitian from the Royal Veterinary College - working with colleagues from the London School of Hygiene and Tropical Medicine and Imperial College - have developed a model to understand how live bird markets act as a "hub" of infection for domestic poultry and if rest days, during which such markets are emptied and disinfected, may reduce transmission.
The results are published today in Interface - the Journal of The Royal Society. PhD student Guillaume Fournié, who led the research, said: "Live bird markets can be a reservoir of infection for domestic poultry and may therefore be responsible for sustaining H5N1 HPAI virus circulation.
"Compared to interventions applied in farms - such as stamping out and vaccination - our model shows that frequent rest days are an effective means with which to reduce H5N1 HPAI infection rates. Furthermore, our model predicts that full market closure - as has been implemented in some countries such as Egypt and Vietnam - would only be slightly more effective than rest days to reduce transmission of the disease."
The models used in the study are based on the live bird market chain in Hong Kong and the analysis restricted to chickens. Fournié, who qualified to be a vet in Alfort in his native France, was awarded fully-funded studentships (tuition fees and a living allowance) in 2007 by The Bloomsbury Colleges - a consortium of six colleges of the University of London: Birkbeck, IoE, LSHTM, RVC, School of Oriental and African Studies, and The School of Pharmacy - to conduct the research.
Since December 2003 outbreaks of H5N1 Highly Pathogenic Avian Influenza (HPA1) affecting domestic poultry have been reported in 50 countries across the world. Massive economic losses and the pandemic threat make H5N1 HPAI one of the greatest current public health concerns.

The collaborative study, which was sponsored by Norbrook, involved each practice blood testing a cohort of six lambs on 25 individual farms from 16 weeks of age, to look for evidence of antibodies to liver fluke.
Only 11 farms had positive results in the period from June, up to December, with most only showing one or two lambs out of the six sero-converting later in the year. Even farms located reasonably close by showed positive cases months apart.
Mark Thompson of Craven Farm Vets in Yorkshire said, “I did not expect to see positive results as late in the year. I expected a positive result earlier (September/October) as the farm is a very wet farm with a long-standing issue with fluke. Also, the weather in the area has been ideal for the intermediate host and fluke development on the pasture.”
Emily Baxter of Drove Farm Vets in Wiltshire said she thinks there is scope to use the results to improve the timing of dosing with flukicides and more responsible use: “We will use the results from this year’s testing to re-evaluate the farm’s fluke control protocol which will help reduce the level of use of flukicides earlier in the season for years with similar weather patterns and assist in planning of strategic grazing.
"We’re looking forward to continuing our work this year, no two years are ever the same, that’s why monitoring is so important. It will allow both ourselves and our clients to build up a picture over time of what’s happening not only on their individual farms, but potentially down to field level! This will allow us to provide more conclusive recommendations. For now, it’s clear that testing has great promise as a means to provide tailored advice to farms on how to control fluke and use the flukicides available to us responsibly.”
Emily says it’s important that farmers do not stop using flukicides based on the results of this study nor assume that there is less risk: “We all know how significant the losses from acute and chronic fascioliasis can be. We therefore encourage farmers to talk to their vet before they change their flukicide treatment protocols and discuss the possibility of using serology as an adjunct to determine how and when they treat.”

FMDMost of you will be aware that IP8 was declared yesterday. This is a farm in the POZ that was slaughtered on suspicion after detection during surveillance visits. This has lead to further slight changes in the boundaries of the PZ and SZ. These were early lesions and initial laboratory results confirm this.
The epidemiology of the spread of FMD in the area north of Egham suggests that cattle on 4 premises in the vicinity of IP8 have been exposed to infection either via possible aerosol or fomite spread and are likely to develop disease. These premises will be culled as dangerous contacts.
BLUETONGUEThe various zones for controlling Bluetongue have now been defined. These are difficult to describe because they use roads etc. as boundaries but detailed maps and description of the boundaries are available at:

When farms straddle a road that is a boundary the whole of the farm is classed as being in the zone. It may be possible to put parts of a holding that is outside the zone as a separate unit with consultation with local Animal Health office.

Defra are worried that farmers are not reporting cases of BT because they feel there is nothing that can be done so why bother. It is still a notifiable disease and there is a need to monitor spread for various trade implications. There are, of course, likely to be welfare considerations with clinically affected cases. These will need to be addressed with veterinary consultation and may involve symptomatic treatment or euthanasia.
The prevalence of the disease in infected herds/flocks is still low with 1:53 and 1:190 being affected on surveillance.
Leading human and animal health organisations – the British Medical Association (BMA), British Dental Association (BDA), British Veterinary Association (BVA) and Royal Pharmaceutical Society (RPS) - have joined forces for the first time to promote a ‘One Health’ approach to ensure responsible guardianship of antibiotics and their proper public use on European Antibiotics Awareness Day tomorrow.
Antibiotics are obviously a vital medicine for both animal and human health, and modern health systems rely on the availability and effectiveness of a range of antimicrobials to treat and prevent infections. Yet inappropriate use – such as prescribing for self-limiting infections, not finishing a prescribed course of antibiotics or habitual prophylactic use in healthy animals to prevent disease - all contribute to the development of resistant bacteria in both humans and animals.
Whilst antibiotic resistance in animals does not perhaps present the greatest immediate existential threat, there is a strong argument for a united message to the public coming from doctors, dentists, chemists and veterinary surgeons.
Quite simply, some 13M households own a pet. Pretty much all of them visit a vet at some stage, and most will be a human patient at some point in their lives. So, what a veterinary surgeon says about the use of antibiotics in animals has the potential to help transform how owners think about them when they come to visit the doctor.
A survey of general practitioner doctors last year found that 90% feel pressured by patients to prescribe antibiotics and 28% have prescribed them several times a week when not sure they are medically necessary. If veterinary surgeons refuse to prescribe them for pets unless medically necessary, explaining that they are a precious resource which can no longer be wasted, it could help relieve the pressure on doctors. The same goes for the importance of compliance; if owners learn the importance of their pet taking the full course, there's a greater chance they'll understand why they need to complete their own course of treatment.
BVA President Sean Wensley said: "Antimicrobial resistance (AMR) is too important an issue for uncoordinated approaches amongst the medical professions. We need to work together to share knowledge and successful approaches, as well as taking responsibility for not just the immediate clinical situations before us, but also for our role in supporting the health and wellbeing of wider society. An effective approach to tackling AMR is about professions and patients working together for the greater good."
BMA board of science Chair Professor Sheila the Baroness Hollins said: "Doctors have expressed significant concern about the threat of a ‘post-antimicrobial age’. It is vital that we see action to improve antimicrobial prescribing across all disciplines. Only by different professionals working together to improve antimicrobial stewardship will we start to address this major public health issue."
BDA President Professor Nairn Wilson said: "All health professionals have a responsibility to draw a line under inappropriate or unnecessary use of antibiotics. One in ten prescriptions in the UK are provided by dentists, so we’re determined to do our bit. We are facing a clear and present danger to public health, and it will only be overcome if we act together."

According to Intervet, a study undertaken in Germany last summer has proven the efficacy of Butox SWISH at killing the midges responsible for carrying the bluetongue virus, when applied monthly to cattle. The study showed that even brief contact between a midge and hair from a treated animal killed midges rapidly for up to four weeks post-treatment.
The standard dose, applied in the usual way (on the neck and topline of the animal) delivers full-body coverage of Butox SWISH’s deltamethrin, helped partly by its unique micronised formulation. The product is also highly lipophilic which means it binds onto the natural oil secretions of the coat helping to improve coverage.
The trial used six heifers (each weighing around 400kg) treated with 30ml Butox SWISH (the standard fly dose and method of application). Two un-treated heifers were also studied for comparison.
During the five week period, contact between midges and treated hair caused immediate paralysis, and the midges then died at varying intervals depending on contact time and the number of days post-treatment with Butox SWISH. The trial verified the application of Butox SWISH monthly at the standard dose for cattle.
Trial director, Professor Mehlhorn suggested that producers should be prepared to reduce vector contact by removing their breeding habitats and combining this with the use of a knock-down treatment like Butox SWISH, which is the only product of its type which has proven efficacy against midges. Used in conjunction with vaccination, these measures may not stop the disease entering the herd, but will help reduce the spread of disease.
Intervet/Schering-Plough Animal Health has launched its complimentary blood testing service that allows practitioners to investigate one of the main infectious causes of early embryo loss in sheep flocks.
The annual subsidised Barren EweCheck diagnostic scheme is available from 1st December 2009 until 31st March 2010 for breeding ewe flocks. The service establishes whether Toxoplasma gondii is potentially implicated in any empty or aborting ewes.
ISPAH says that data from the 2009 scheme showed that 69% of flocks had been exposed to the toxoplasma parasite at some point before blood sampling. As a result, practitioners were able to give clients receiving a positive diagnosis timely advice on disease control and the benefits of a Toxovax vaccination regime.

Novartis Animal Health UK has announced that it is committed to maintaining a reliable supply of Bovidec BVD virus vaccine throughout the coming season.
The announcement comes following concerns surrounding the availability of BVD vaccines. Novartis Brand Manager Eugene Smyth said: "We have been in discussion with the regulatory authorities to try and ensure we can meet the demand over the coming months and we have a large team of territory managers and technical services veterinarians dedicated to supporting our customers through the season".
Bovidec BVD virus vaccine, immunises adult cattle against the BVD Type 1 virus and for active immunisation of calves against BVD Type II virus.
London's Savile Row was turned into a sheep field today, to highlight The Campaign For Wool; a coalition of industry groups convened by HRH The Prince of Wales to educate consumers about the benefits of wool.
As the sheep grazed, the Row's famous tailoring houses held events where the public could find out more about the natural, sustainable qualities of wool.

Animalcare has launched Florgane, a novel, patented, long acting florfenicol 300mg/ml suspension which the company says has significant advantages over existing organic solvent-based florfenicol products on the market when used for the treatment and prevention of bovine respiratory disease (BRD).
Florgane is an aqueous, low viscosity advanced formulation, which, claims Animalcare, means that it is easier to inject even at temperatures as low as 5°C. Low temperature syringeability is important, as a large number of calves may need to be treated during colder weather at a time when the challenge from infectious agents is greatest. A syringeability study apparently demonstrated that Florgane is up to five times faster to administer when compared to the leading florfenicol non-aqueous formulation.
A paper on the treatment of naturally occurring respiratory disease with Florgane was presented recently at the British Cattle Veterinary Association (BCVA) Congress in Torquay. According to the company, the multi-centre field studies in Belgium, Germany and Italy, in over 400 cattle with bodyweights ranging between 73-180kg, demonstrated that Florgane given by a single intramuscular injection, at a 25% lower dose of 30mg/kg, is as efficacious as non-aqueous florfenicol 40mg/kg administered by subcutaneous injection, measured by treatment success and relapses.
Post mortem examination at day 27 (meat withdrawal of 37 days) confirmed that, despite being given via the intramuscular route, Florgane is well tolerated at the site of injection due to its aqueous base and physiological pH. Maximum volume per injection site is 15ml allowing calves up to 150kg to be treated.
Animalcare adds that a further advantage of Florgane is that it persists in plasma above the MICs for the three most common pathogens for significantly longer than conventional extended action 300mg/ml florfenicol solution preparations - thereby prolonging bacterial killing time. Therefore, owing to its improved bioavailability and excellent pharmacokinetics, Florgane can be given at an easy to remember dose of 1ml per 10kg.

The British Occupational Hygiene Society (BOHS) has announced the publication of a book on controlling skin exposure and wet work. The book aims to fill a knowledge gap about dermal exposure risk management in workplaces which are particularly affected, such as veterinary practices.
The consequences of a failure to adequately control dermal exposure to hazardous agents do not always become apparent until years after exposure, and because of this, many employers are either ignorant of the consequences or ignore the potential for ill-health until it is too late. In Great Britain, over 29,000 people are suffering from work-related skin disease and every year over 3,500 new cases are reported. Alongside this, many workers are unaware of the ways in which skin comes into contact with chemicals, or how dermal exposure can contribute to inadvertent inhalation and ingestion exposure.
Healthcare workers are susceptible. Vets and nurses are, for example, at risk from disinfectants, detergents, wet-work, latex gloves, anaesthetics, antibiotics and antiseptics, phenothiazines, formaldehyde, glutaraldehyde and liquid chloroxylenol.
The book provides an introduction to the structure and functions of the skin, information about the diseases caused by dermal exposure and the occupations and chemicals involved, regulatory requirements related to dermal exposure at work, exposure and risk assessment, and risk management strategies. It is both comprehensive and accessible, and clearly illustrated with photographs, figures and tables throughout, ensuring that it will be of practical use. Virbac Animal Health has launched a special scheme to encourage farmers to have milk samples analysed in line with the industry-wide campaign to promote the responsible use of antimicrobials.
Under the terms of the scheme, veterinary surgeons purchasing Virbac's Cephaguard DC therapy will receive vouchers to distribute to their farmer clients. These vouchers can be redeemed for milk bacteriology to determine the pathogens causing mastitis and so ensure treatment is more targeted and effective.
Brigitte Goasduf MRCVS, Ruminants Marketing Manager at Virbac, said: "Mastitis is a major concern for dairy farmers with reported numbers of cases ranging from 26-50 for every 100 cows.

Defra's Chief Veterinary Officer's annual report was published this week, reviewing developments in animal health and welfare during 2007.
The report, by former Chief Veterinary Officer (CVO) Fred Landeg, summarises progress and developments in the field of animal health and welfare within Great Britain. It also provides a continuous statistical record of the state of animal disease in Great Britain.

Life Technologies has announced the launch of a new ELISA test for bluetongue virus (BTV) antibodies.
According to the company, studies have confirmed that the LSI VET Ruminant Bluetongue II-serum ELISA can detect antibodies to BTV in experimentally infected sheep after 6 days, and in cattle after 7-8 days - several days sooner than some other commercially available test kits, including the company's previous ELISA test which the new product has replaced.
Life Technologies asked the UK's Pirbright Institute, the European Union Reference Laboratory for BTV, to conduct a preliminary assessment of the new test. The new test was able to detect antibodies in all 24 BTV reference sera and BTV 26 held by Pirbright, and showed high sensitivity in a longitudinal study of experimentally infected animals.
The Pirbright study also demonstrated that the LSI VET Ruminant Bluetongue II-serum ELISA could detect BTV antibodies in cattle and sheep that had been vaccinated with an inactivated Bluetongue vaccine.
The new test was developed by Life Technologies Animal Health in its research laboratory near Lyon in France and will be available in EU countries via Life Technologies Animal Health distributors according to local regulatory requirements (see www.lifetechnologies.com/lsi-animal-health) for details.
Defra has announced that Great Britain is to be declared bluetongue-free from 5th July 2011.
From this date, bluetongue susceptible species being exported from Great Britain will no longer need to be vaccinated against BTV8 or meet any of the other 'Annex III conditions.
However, the current EU Bluetongue Directive only allows bluetongue vaccination within a protection zone, so from 5th July vaccination will no longer be permitted in Great Britain.
The British Veterinary Association is supporting Defra to lobby for these rules to be changed to allow vaccination in bluetongue areas.
Harvey Locke, BVA President, said: "The move to bluetongue freedom is thanks to the excellent partnership between government, farmers and vets across Great Britain. It is a tremendous achievement to turn the situation around from the outbreak in 2007 to freedom in 2011.
"We are disappointed that changes to the EU Directive to allow vaccination to continue could not be made before the deadline and we will continue to work with Defra to try to secure this change as soon as possible to allow vets and farmers to make the right choice to protect their herds.
"Those farmers and vets that wish to vaccinate must do so before the 5th July.
"The BVA would reiterate the message to farmers to source stock responsibly to help avoid a future outbreak."
The review coincided with a move last month from RUMA, the agricultural and food industry alliance which promotes responsible use of medicines in farm animals, to align with the EMA position.
The announcement means that under PVS guidelines, fluoroquinolones, 3rd and 4th generation cephalosporins and colistin will retain Class 3 status. This means they are only used when no other options are available and when supported by laboratory sensitivity testing – or exceptionally when all other options have failed.
Class 1 antibiotics form the standard prescribing list within responsible use guidelines and Class 2 are used only when sensitivity tests or clinical experience proves standard antibiotics are not effective. In all treatment decisions the health and welfare of the pigs must always take priority.
Betalactams with betalactamase inhibitors (clavulanic acid) were previously in the highest priority Class 3 but in light of the list defined by the EMA, which has been adopted by the Veterinary Medicines Directorate as well as RUMA, the PVS has reassessed the risk and moved them to Class 2 to align them with plain betalactams. This means they are still subject to voluntary restrictions, but their use would not necessarily be only as a last resort.
PVS president Mark White said that despite the ongoing review process and the potential to reclassify certain products, the Society’s underlying principles remain unchanged: "The total amount of all antibiotics used on pig farms to treat and prevent disease should be minimised, and the types of antibiotics used in pigs should be considered in light of the implications for antibiotic resistance challenges in both pigs and man.
"This policy is proving successful with AHDB Pork’s electronic pig medicine book (e-MB) recently showing that use of colistin appears to have fallen by over 70% in 2016 following its move to a Class 3 antibiotic in late 2015.
"We must also continue to be alert to the possibility of cross resistance, even in older antibiotics that have little or no use in humans. Risk must be acknowledged where it exists and overall reductions are still imperative."
RUMA has welcomed the review and alignment from the PVS. Secretary General John FitzGerald said: "Farmers and vets need boundaries within which to operate and clear objectives to meet, and we would urge all supply chains, from farmer to retailer, to align with the sector position on best practice use of antibiotics, as defined by each sector’s professional veterinary organisation."
The Society of Practising Veterinary Surgeons (SPVS) is putting out a last call to take part in the SPVS Salaries Survey 2015, open to everybody working in the veterinary profession.
The long-standing survey provides a benchmark for veterinary surgeons, nurses and support staff to see trends in pay and conditions. Designed to provide a complete picture of remuneration levels, the survey covers basic earnings, bonuses, dividends, overtime and benefits such as accommodation, car allowances, private medical insurance and pensions. It also includes questions on employment contracts, hours worked, out of hours and weekend working.

I know it is also the go to reference guide when practices are recruiting to help ensure they are offering competitive salary packages.
