Equine obesity is a growing problem not just in the UK but around the world. The debate explored how various sectors of the equestrian industry see the challenges faced by people who are struggling to manage their horse’s weight. It included the views of equine vet Lucy Grieve, horse owner Helen Gale, livery yard owner and coach Beth Maloney, nutritionist Liz Bulbrook, competition judge David Ingle and equine welfare officer Penny Baker.
The session was chaired by equine vet and nutrition specialist Professor Pat Harris with the support of Dr Tamzin Furtado, a social scientist with a special interest in human behaviour change for animals.
The panellists shared their views on what they believed to be the blocks for horse owners in recognising obesity and addressing the problem.
They identified the importance of empowering owners and working with them to make step by step changes with the support of a reliable and trusted team including vet, nutritionist, yard owner and farrier.
Lucy said: “Vets are in unique and privileged position to provide an achievable and effective tailor-made strategy for individual horses.
"We must all learn to overcome avoidance and engage recognise, accept and understand, and tailor a solution.”
Helen added: “Owners need to be able to recognise and acknowledge that their horse has a weight problem and be motivated to take action. Communication is key. It’s important for others to understand why you are following the advice form vets and other experts and how they can best help."
There was recognition that a bespoke approach is needed for every horse, owner and situation. Some livery owners may face challenges in changing management practices and peer pressure on a yard may prevent people implementing intake restriction measures such as a grazing muzzle or restricted turnout.
Beth said: “Peer pressure can be daunting and there can be a lack of confidence that you are doing the right thing."
Communication and proactive signposting to reliable advice and scientific information were identified as pivotal, not only in helping to identify weight changes but also to build horse owners’ knowledge and confidence. This would build a clearer understanding of the calorific value of feeds and of grass and forage, what constitutes a balanced diet, the importance of regular exercise and weight monitoring, being able to identify the difference between fat and muscle and being familiar and comfortable with the range of weight control tactics, such as grazing muzzles, restricted grazing and track systems.
The language used was also identified as important; gauging the needs and response of the individual and asking the owner what they think, would be more effective than using potentially inflammatory language - for example choosing the phrase ‘fat pad’ rather than the terms ‘overweight’ or ‘obese’.
Lucy said: “If we are all being really honest with ourselves obesity is neglect. I doubt many vets would walk away from an emaciated horse or an infected wound or a cloudy eye so why are we so willing to walk away from these ticking time bombs which are actually a real welfare issue as the animal is potentially at risk of losing their life?”

Duncan, an 85kg miniature horse, was first admitted to the Philip Leverhulme Equine Hospital in July this year for investigation of severe hind limb lameness. A CT scan revealed that Duncan had dislocated the left hip joint and damaged it irreparably.
Equine surgical specialist Dave Stack said: "Treatment options for this problem are very limited, especially with extensive damage to the head of the femur.
"I discussed Duncan’s predicament with two small animal surgical specialists, Professor Rob Pettitt and Andy Tomlinson, who agreed that performing a total hip replacement offered Duncan the best chance for recovery."
The procedure had been attempted in small ponies before, however, all known previous attempts had failed. The surgery required careful preparation and the combined knowledge of specialists in both small animal and equine surgery, as well as colleagues from the anaesthesia and internal medicine departments.
Working together, the teams were able to replace Duncan's left hip using implants designed for use in large dogs.
Equine Surgical Resident, Matthew Cullen said: “Although always complex, hip replacements are relatively common in dogs, so the experience of the small animal surgeons was absolutely vital as Duncan presented a highly unique challenge.
"Despite that he has made an excellent recovery and was able to walk and trot almost normally at his last check up!”
Duncan was hospitalised for over three weeks and required round-the-clock supervision in the first few days after surgery. As he grew stronger, physiotherapy formed a large part of the postoperative care to help him get back on his feet.
Rob Pettitt, Professor of Small Animal Orthopaedics at the University’s Small Animal Teaching Hospital, said: "The opportunity to provide Duncan with a normal life using a procedure that we perform regularly in dogs but that has never been successful longer term in equids was a unique experience. Our role as surgeons was just a small part of the huge teamwork that has resulted in this successful outcome."
Dave Stack added: “I would like to express my gratitude to all who contributed to the success of Duncan’s recovery, not least Rob Michael of Thompson House Equine Clinic, Duncan’s vet at home, whose care of him has been invaluable. I am thrilled that Duncan will live a comfortable life and delighted that Duncan’s owners have the opportunity to continue to spoil him for many years to come."

The College says this will pave the way for new diagnostic tests for what could be one of the most common causes of pregnancy loss in mares.
The researchers, led by Dr Mandi de Mestre (pictured right), Reader in Reproductive Immunology and Head of the Equine Pregnancy Laboratory at the RVC, collaborated with seven different veterinary practices to gain access to samples from across the UK and Ireland and found that around 20% of the pregnancies lost were aneuploid, which is when a copy of a whole chromosome is either duplicated or lost (similar to Down's).
Charlotte Shilton, RVC PhD, the student who performed the analysis, applied three different genetic approaches to confirm the results.
Work is now underway to identify the underlying cause of these aneuploid pregnancies, with early data from this study suggesting it is most commonly introduced via the egg or sperm. Until now, chromosomal defects such as aneuploidy have only been reported as a rare condition in young horses with developmental disorders.
The researchers say the study explains why the condition is so rare in horses, with most embryos and foetuses possessing this genetic change dying very early in development, as is also observed in human pregnancy. The study highlights the need to reconsider this genetic condition both in pregnancy loss but also for early developmental disorders.
Dr Mandi de Mestre, Reader in Reproductive Immunology at the RVC, said: “Early pregnancy loss remains a very frustrating condition for clinicians to treat as the underlying cause is unknown in around 80% of cases. These findings will allow researchers to develop new diagnostic tests for pregnancy losses, which would offer hope to thousands of owners of breeding mares that suffer this condition.
“A diagnostic test would allow them to make informed decisions on treatment strategies and to advise on whether they should invest in further attempts to breed their mare benefiting both horses and their breeders alike in the future. I would like to thank both the Thoroughbred Breeders Association and our collaborators at Texas A&M University and the participating veterinary surgeons for their support on this project.

The new resources include the Equine Veterinary Journal’s (EVJ) publication of BEVA’s Primary Clinical Care Guidelines on wound management in horses together with BEVA’s first ever digital CPD course: 'How to Heal Wounds – a masterclass for vets and vet nurses'.
The new guidelines cover three key areas: wound lavage and topical treatments; wound debridement and closure and therapeutics for wound healing.
Sarah Freeman, who led the project, said: "These guidelines are aimed at the vast majority of our membership who are primary care practitioners. Topics covered are voted for by BEVA’s panel of specialists and represent a benchmark in time against the strength and weakness of evidence that support it."
The wound management guidelines follow BEVA’s recommendations on the safe use of analgesia in horses, which were published earlier this year.

With fewer than 72 female Suffolk Punches remaining in the UK and fewer than 300 in the world, every female born is vital to the survival of this endangered British horse.
In 2019 Tullis Matson, owner and managing director of Stallion AI Services, saw an opportunity to use a new technological advancement in the sex sorting of equine semen to provide a lifeline to Britain’s critically endangered rare and native horses.
Tullis said: “To be able to use our reproduction expertise in this way, to help preserve an irreplaceable part of our magnificent heavy horse heritage is something we have been working towards for many years. The challenges have been great and many, but watching the birth of this beautiful, healthy filly foal was a truly magical experience."
The sex sorting project uses specialist equipment to sex sort the semen prior to insemination based on the difference in DNA content between X and Y bearing spermatozoa.
Ruby, the Suffolk Punch mare and Holbeach Iggy, the Suffolk stallion were selected and matched based on their genetics as part of a project between The Rare Breed Survival Trust and Nottingham Trent University that uses pedigree information to enable the breeding of small populations in a way that minimises the risk of inbreeding and genetic decline.
Christopher Price, Chief Executive of the Rare Breed Survival Trust, said: “This is tremendous news for anyone concerned with the conservation of our native equines. The most effective way of increasing the population size of this very rare breed is by increasing the number of fillies being born.
“The project demonstrates the viability of using new techniques for selecting female foals in order to increase the breeding population much more rapidly than could be achieved through relying on traditional methods. We hope it will prove to be a model for more projects in the future.”

The company says the new product has been 10 years in development and uses 'Soft Mist' technology - developed originally for human use by its pharma business - to deliver medication deep into the horse's lungs.
The active ingredient in the Aservo EquiHaler is ciclesonide, a corticosteroid that is activated directly in the lung, reducing the lower airway inflammation associated with severe equine asthma and reducing the risks commonly associated with systemic corticosteroids.
The Aservo EquiHaler does not use a propellant, so it does not contribute to greenhouse gas emissions. Boehringer also highlights that it is made from up to 50% recycled materials. In April, the product was awarded a Red Dot Design Award.

The company says Dormazolam works in synergy with other induction agents to provide an extended duration of anaesthesia without adversely affecting the quality of recovery 2,3,5.
Benzodiazepines are commonly incorporated into anaesthetic regimens to alleviate issues seen with the administration to ketamine alone2,3,4, including inadequate muscle relaxation2,3,4, anaesthesia not lasting long enough2 and induction failure2.
Despite the issues caused by the sole use of ketamine, it remains the primary anaesthetic induction agent of choice for more than 90% of equine vets2.

Squamous cell carcinoma affects horses’ genitalia, eyes, or skin around the eyes. The tumours typically have a guarded to poor prognosis, and the therapeutic approaches available have variable success rates. Euthanasia on welfare grounds is necessary in a significant number of cases.
Before this new research was published, pathologists and researchers Dr Alejandro Suárez-Bonnet and Professor Simon Priestnall, both from the RVC, had already undertaken research which showed that equine squamous cell carcinoma affecting the penis frequently acquires the ability to degrade the extracellular matrix and become much more biologically aggressive2. This is known as ‘epithelial to mesenchymal transition’.
This new study, recently published in Scientific Reports, is the product of a multi-institutional effort, which used both “classical” anatomic histopathology assessment and artificial intelligence to demonstrate a correlation between chronic inflammation, equine papillomavirus infection and progression of equine penile squamous cell carcinoma.
Several additional cancer-related signalling molecules, which are important in researchers’ understanding of human penile cancer, were also studied and the results obtained will lead to a much better understanding and treatment of the cancer.
Simon Priestnall, Professor of Veterinary Anatomic Pathology at the Royal Veterinary College, said: “Equine squamous cell carcinoma is the second most common type of cancer in horses, with a variable prognosis, so we are delighted to have made this breakthrough that provides a greater understanding and can lead to more effective treatments for horses and their owners.
"The RVC has always recognised the value of collaboration, and Alejandro and I enjoyed working alongside the teams at KCL, the University of Edinburgh and UCL. This project shows what can be achieved when different organisations work together with a common aim and we hope the similarities between the tumour in horses and people can offer a true One Health benefit."
Dr Alejandro Suárez-Bonnet, Lecturer in Comparative Pathology at the Royal Veterinary College, said: "I am so pleased with the results of this study, which will hopefully pave the way for improved prognosis of horses diagnosed with equine squamous cell carcinoma.
"Our work is not over, however. Thanks to the help of a grant from the Horse Race Betting Levy, Simon and I are leading a Master’s degree project investigating equine penile, ocular and vulvar squamous cell carcinomas from a broader perspective.
"Our hope is to identify if variability exists between equine papillomavirus infectious status, morphological and microscopic difference, and cancer progression, with the aim of identifying potential therapeutic targets as well as diagnostic markers with prognostic implications for maximum benefit to the British equine population.”

The apple-flavoured, gastro-resistant granules can be mixed with dry feed to make administration easier for owners.
Animalcare says that up to 37% of leisure horses and 93% of race horses are reported to be affected by gastric ulcers1, for which the most prescribed treatment is omeprazole.
Equizol’s granule formulation has been developed to support owners who find syringe administration difficult or wasteful. It is presented in packs of 28 sachets, each containing sufficient omeprazole to treat 200 kg body weight. Owners mix the appropriate number of sachets into a small amount of the horse’s feed.

The charity says that it had already been suffering a period of 'dire financial constraints' and the economic effects of coronavirus are now having a direct effect on funding.
The charity's Trustees and Executive Committee is still trying to find emergency funding to try and save the organisation, failing which it will close on the 31st March.
The two-day collaborative workshop was hosted by the Jockey Club Estates, Newmarket and sponsored by The Gerald Leigh Charitable Trust and the Beaufort Cottage Educational Trust, together with a number of other industry contributors.
Injuries to the racehorse fetlock are rare: it is one of a number of racecourse musculoskeletal injuries which are collectively estimated to occur in around 8 of every 10,000 race starts in the UK. Nevertheless, fetlock injury can be extremely impactful and can be career-ending in some injured horses.
On the first day of the workshop an expert panel discussed how diagnostic imaging prior to racing can contribute to risk reduction.
Radiography is currently the most widely available tool used to identify tiny fractures which will heal effectively, providing exercise is reduced. However, advanced imaging such as standing MRI, standing CT and PET scanning, have the potential to identify pathology even earlier. At the workshop, the expert panel reviewed existing knowledge and discussed how to generate the research evidence, which is essential if these novel technologies are to gain a place in effective pre-race risk assessment programmes.
The following day a larger group of stakeholders reviewed the expert panel’s conclusions and discussed the need for greater transparency, education and communication amongst the racing industry stakeholders, all of whom share responsibility for ensuring racing continues to collaborate and enhance racehorse safety and welfare.
Prof Celia Marr, Editor of Equine Veterinary Journal, who chaired the meeting said: “Racing has an excellent safety record and the injuries we are talking about are extremely rare. The low prevalence of fetlock injury makes it very difficult to pinpoint the affected individuals. But it is essential that we continue our efforts to do so ever more effectively because if silent injury is not detected it can progress to become much more serious."
Pete Ramzan, Partner at Rossdales LLP, who co-ordinated the workshop said: “There was a great need to get some of the key experts leading these new technologies together in the same room to correlate their findings and work out how to translate them into tangible reductions in serious injury rates. One of the somewhat unexpected outcomes of the discussions was that despite the fact that we are riding the crest of a wave of technological advances, basic radiography still has much to offer; better education around the application and interpretation of radiographs has real potential to allow vets like myself at the coal face to detect injuries at an early and recoverable stage”.

However, despite keeping this POM-V drug to hand, only 7% said they would seek advice for minor wounds and over 25% said they would not contact the vet even if their horse was lame.
This, says the company, raises concerns about the appropriate use of bute in wounded horses and suggests that practices should be providing first aid training for horse owners, covering basic wound management and when to seek veterinary advice.
The survey, which was posted on social media groups for horse owners, showed they were most likely to call out a vet if their horse had a deep wound (96% of respondents). However, only a third would seek veterinary advice for a wound that is bleeding.
Vita, which makes the veterinary-exclusive Omnimatrix skin cream, said that 84% of owners keep a topical treatment for minor wounds in their first aid box, but some use barrier creams that do not provide an antiseptic environment.
Tara Evans RVN, Head of Sales at Vita, said: "It is great to see that owners are equipped with a first aid box for their horses. However, our survey highlights that many are not seeking veterinary advice, even if their horse is lame. This raises concerns over horses receiving appropriate treatment in a timely manner. There is an opportunity for practices to review first aid boxes with owners to ensure they're stocking appropriate supplies, and discuss appropriate wound management."
