The issue of clients obtaining/using medications without prescriptions was raised and I was impressed by the vigorous response of the VMD representative who urged us to report any such findings to them for immediate investigation.

This week a client turned up with a new puppy. The puppy had been vaccinated by the breeder using an american vaccine, sold to a new owner and taken to a vets for the second vacc (they did this and signed the card as a completed vacc course!). The pup was then returned and sold again and presented to us by the new owners.

We were concerned about the breeder having used this vaccine and phoned the VMD. They couldnt have been more disinterested. They basically said that they could only look into it if the new owner was prepared to take it to court! We have a peice of paper with all the breeders details on it, the details of the vaccine and signed by the breeder and they dont even want to see it or speak to the person concerned.

We then spoke to the vaccine company who said "yes it happens all the time, nothing we can do about it"

Am appalled that noone seems to care about this, what sanctions are there to stop people doing this if no one is going to bother to do anything about it!

client led referrals for "alternative" things

How do you guys proceede with these? Have no problems with referring clients for all sorts of things but lately seem to be getting a spate of clients looking for referral for alternative things when we havent seen the animal recently or for that problem or have a diagnosis.

For example had a form faxed through today for Galen myotherapy for a dog with "generalised stiffness backend", we havent seen the dog recently for this (or ever for this) but the dog does have a history of degenerative disc disease so I would have thought not an ideal candidate fior massage?

So dont want to be awkward but feel reluctant to sign a form "referring and authorising" treatment to someone I dont know, have no idea what they are going to do etc etc

Not wanting to pick a fight on alternative therapies just general principles!

What do you think of the 'blessed sleep' protocol for pre euthanasia sedation?

After a less than successful pre euth sedation with dom and torb, it worked but there was some retching and then a lot of agonal breathing, I have been researching better solutions.
I came across an American protocol called blessed sleep, a mixture of ketamine, xylazine, acp and butorphanol.

it sounds great as it’s a small volume with a s/c injection.

for ease they premix and keep a ready made solution

thing is I’m not sure how you would square that with ketamine regulations. We could work out per ml ketamine and log as used but the 10 ml we use to make the solution would be flagged up as being past 28 day breach and need disposal etc.

obviously we could make up the injection each time but that’s a right faff and negates having a simple solution.

anyone in the uk that does this or have any better ideas ?

“Behaviourist “advising drug therapy

Have been helping a family with a resource guarding golden retriever pup. Basically a nice dog with very novice owners and all getting a bit out of hand . I referred them to a clinical behaviourist we have used before and discussed with them that if she felt it necessary, I would prescribe medication for the short term , something like selguan or possibly fluoxetine

yesterday they called and asked to be prescribed Xanax because the behaviourist had said that was better. I immediately asked them to ask for them to send me a report.

they have actually used a different person , with off the shelf type training qualifications, he sent me an email saying that he would prefer Xanax , as “ they “ have found it useful as works more quickly than fluoxetine.

thus would be a completely inappropriate use of this drug and I feel he should not be advising or discussing medication with clients as he has no relevant qualifications.

I'm old enough and wise enough to just tell him no but it worries me that a less experienced younger colleague might take him at face value or indeed that a client might source some from a human stash.

i would like to report him to someone but not sure who , would Royal College be appropriate? Ideas on a post card please !

Aural haematoma technique

A friend posted this link to a paper with new aural haematoma technique which uses subcut sutures.
